State of the Digital Health & Value-Based Care (VBC) Market

Between the influx of A.I. solutions, policy changes, acquisitions, elections, and new ventures, 2024 has proven to be yet another year of innovation, financial discipline, ambiguity, and unyielding optimism within the broader digital health landscape.

Value-based care has emblematized all of these core themes and we at Conduce have experienced our fair share of macroscopic tail- and headwinds throughout this year. Holistically, 2024 was a year of solidifying specialty care as the next exciting frontier in VBC. Our competitive landscape — including the likes of BridgePointMD, Switchboard, Enlace Health, etc — continues to broaden resulting in heightened awareness and interest from the market.

Before we look ahead at 2025, we want to look back and share a few key highlights from this past year.

2024 Year in Review

Operation Highlights

Excitingly, 2024 marked the year in which Conduce evolved from just an idea to a fully-fledged marketplace. In January, operations were formally live in South Carolina with our key primary care enablement partner, Pearl Health, and our network of >150 Nephrologists.

We have been incredibly pleased to see well above average physician adherence to our signals, a designation of clinical relevance for our product, and a trend towards total cost of care savings relative to actuarial projections.

Product Developments

2024 has been a busy year on the Product and Technology team! We started by automating and streamlining our data pipeline. This allowed us to build out the metrics and monitoring dashboards to track financial performance consistently during the year. In establishing a data warehouse in Snowflake, we enabled customization in reporting and analysis, which was an important prerequisite to being able to perform our data science work.

Our talented development team turned our Referral Engine into both an API as well as an application UI and the product became operational  for nephrology patients in early 2024. Expanding the Referral Engine to support Cardiology was completed in November and we are very excited to start serving a broader set of patients in 2025 with it.

A product update would not be complete without a discussion on A.I. We’re pleased with the progress that we’re making in transforming our rules-based algorithms to a machine-learning model utilizing 282 features, predicting cost increases and disease progression. This new model is twice as effective.

Growth & Marketing

Since our emergence from stealth in March of this year, our team has intensely focused on building a strong pipeline to deliver both near and long-term growth. Through these efforts we have engaged leading value-based care organizations and developed over 20 qualified opportunities now accelerating through the sales cycle. The response and receptiveness of the market has been overwhelmingly positive, giving the team confidence as we head into 2025.

Building off the foundation we’ve set in the recent quarters, expect to see the Conduce brand come to life as we make efficient investments to grow reach, awareness, and engagement in the market. Plans for the near year include an exciting brand launch, expanded thought leadership efforts, high-value event marketing, and more

Team Overview

Core to what makes Conduce an impactful company is our team and we’re incredibly proud of how much we’ve grown. After completing our Seed round in March of this year, we were positioned to transition many part-time roles into full time hires creating more continuity and a strong team culture.

We welcomed five new full time employees across our product and growth verticals to support our efforts towards achieving Series A milestones. Between our formal offsites, weekly lunch and learn seminars in which members present and teach on a desired topic, and the many informal interactions required to operate the business this team has proven to be talented, cohesive, and passionate about driving innovative results in healthcare.

2025 Outlook

Our mission for 2025 is clear—grow the business, prove and advance our offering, and raise the capital required to continue building a strong and scalable foundation for a generational company in value-based care.

We are incredibly excited to expand our operations from Georgia and South Carolina into New York state with Pearl Health, and launch new partnerships in those and other markets. Strategically, the Northeast has always been an advantageous geography for us and we are expecting meaningful results.

In line with partnership expansion, we will extend our multi-specialty focus by launching a new set of specialties starting with oncology. The patient need and partner demand made oncology care the next logical inclusion into the Conduce marketplace. This will allow us to impact more patients in one of the most vulnerable care settings across new and legacy partners.

Thematically, this is the year in which Conduce’s partnerships will grow, positive patient outcomes will be achieved, and the incorporation of A.I. and novel technologies into our product will crystalize. Thank you again for being a part of this special journey into healthcare innovation as we build the bridge to value-based specialty care.
